It depends on the laws of your state.
In some states, your name can be changed (with or without your consent) by your parents until you are emancipated. (generally 18, but in at least 1 state, 19))
In other states, once a birth certificate has been issued, only a court can change the name upon a proper application. If you do not like your given (first) name, and your parents agree to the change, I would recommend that you consult an attorney in your state on the process for change. If your parents object, and you are still under the age of emancipation (in your state), then you may need to wait till that age.

Yes, but your parents must consent to the change. You have to re-fill out all of your records-birth, insurance, school, anthing like that. Go to a county court house and ask for the paperwork.
