Many things come to mind.
Do you buy your gas at the same place? Have you ever had trouble like this with any other vehicle you filled there? Maybe he has water in his gas and either doesn't know or doesn't care.
That's only a couple. Smoke fro the exhaust doesn't necessarily mean gas problems either.
Maybe your timing is slightly off. Maybe you have a fouled plug. Maybe you have a clogged catalytic. You did say it is a 1990 so any of those can cause that.
Tryan additive that gets rid of water in the gas and then fill it up at another place that sells your type of gas.
Other things to think of.
What colour is the smoke?
If it's just white then it could be just condensation buildup. If it has any tinge of blueness to it, then it is most likely oil related. If it's hot where you are, and you run the car in a lot of stops and starts, then try 10W30 instead of 5W30 or even 10W40 instead of 10W30.
I could talk for hours about "smoke from the exhaust", but quite often youcan tell by the colour and amount what is going on.
My guess on the little info you gave me? Water in the fuel.

check your oil level, if it too low then you could have scored a cylinder. Resulting in oil passing into the comustion chamber and burning with the gas and making smoke out of the tailpipe

If you have a bad fuel injector it will rattle like spark knock and will produce smoke from the exhaust.

if the smoke color is white your engine is (bye bye)
but if it kind of gray or black it is a fuel problem and you will notice that by the mpg.
I hope it is not the engine cause I know these Mercedes are very good and they go forever.

as far as the noise it could be what they call desieling if thats the case throw in some octane booster that might help (it wont hurt) . as far as the smoke it could be a number of things .. bad valve , bad ring , sensor, etc .....

Could be oil getting into the combustion.. That's about it, with what you are describing...
Have a mechanic check it out...that's the only way to be sure.
