I can't imagine it's just old bats, because I'm sure they use new bats most of the time. Does it have to do with the way a batter is holding the bat or what type of pitch it was?

It has to do with how the pitch hits the bat. If you watch closely, you'll see that bats usually break along the grain of the wood. It's simply a matter of the ball hitting the bat hard enough and at a place that causes a crack along the grain. Keep in mind that a baseball thrown in the majors can be moving up to 95 mph - concentrated in the fraction of a square inch that makes contact with the bat and you have a lot of force working on that bat.

If you hit the ball with the bat on thelaminated edge, that can cause enough stress to break the bat.

Hitting it too low on the bat (the narrow portion) can have the same effect.

inside pitches. especially cutters or sliders moving in on hitter hands. the thinner the spot where the bat the more likely.

I don't know but it used to annoy the **** out of me. I would hit someone's skull a certain way and the stupid bat would splinter in my hands. I switched to metal and it solved the problem.
