Depends on how much respect you want to bestow on the person you are asking......

>For older people, and seniors, you might use Aap

"Aapkaa naam kya hai?"
(your name what is)

>For equals, and for juniors, use Tum.

"Tumhara naam kya hai?"

>Tu is rather informal, and discourteous and outright rude, used only with close friends, by some people. Refrain from using it

Certainly, if you are meeting someone forthe first time, you can notprobably use this, bcosas I said, it might be considered outrageously rude.

"Tera naam kya hai?"
