I have tried everything to do that too.

I found the only way is to go to tons of random sites. Since everytime you go to a site the name moves down one. Keep going to sites until the one you want to disappear is gone.

It depends what kind of toolbar are you talking about. If it's a yahoo or google toolbar you can do that easly from toolbar properites. find the little downpointing arrow near toolbar's logo, go te prefferances/settings find the appropriate section and erase the history. Toolbars usualy will support a history-disable feature. Look for it in the toolbar settings.

Right click on it and select Delete.
