One of the most-missed problems is poor airflow...First, make sure the condenser is debris-free, and that the fins are not all bent over. The condenser is located right in front of the radiator. Bugs, rocks, plastics bags, etc. covering the front of it prevent air from passing through, cooling not only your radiator, but also the refrigerant in the condenser.
All engines have either a fan clutch or radiator / condenser cooling fans. Fan clutch go out all the time. The fan clutch is located between the radiator and the engine, and has a big fan blade attached to it. With the engine on, that fan should be turning very fast, and moving a lot of air, TOWARDS the engine, If the vehicle has an electric fan, make sure it (or they) comes on, and that the air is fast, and moving TOWARDS the engine. If the fan(s) does not come on, check the fuse. If that's not it, you may need to replace the fan.

The only way to know for sure whats going on with your airconditioner is to know your high and low side pressures. Have you had gauges hooked up to them? If not, you can't know if it's charged properly or even if the compressor is doing it's job. If your a car savvy do it yourselfer, I'd recommend buying an inexpensive set of gauges. Typically, a 134a system will run a low side pressure of 30 - 40 psi with a high side pressure of 225 to 300 psi --- depending on the outside air temperature. Knowing what your operating pressures are would answer most questions about what is wrong and what's needed.

Could be that the system is working fine, but the damper on the duct work in the car is not operational. Check the control to find out if it's vaccum or electronic. Then go from there. Usually there's a vaccum line unplugged or a motor assy is bad if it's electric. Good luck.

How do you know the refrigerant is good?

It could be a temperature sensor.
