It depends you are in which hospital in which country. In a hospital of the developed country nurses keep records of the general examination like anaemia, cyanosis, jaundice, temperature, oedema,pulse, respiratory rate, even blood pressure, maintain intake output chart of some patient.(Positive findings are to be noted in the history sheet). Doctors gives the treatment, add or omit medication in the treatment sheet and advice for the required investigation. Confirm diagnosis and the D/D also written by the Doctor.
