I am looking for the origination of GIGO. The first person to write it down, speak it at a conference or chastise a coworker with it. I hope to find the name of the person, the date it originated, and the reason it was quiped.

It looks like it was first used in 1963 in an IRS bulletin - see below.

GIGO is usually said in response to users who complain that a program did not "do the right thing" when given imperfect input. The first example of this was probably cited by Charles Babbage, inventor of the first programmable device who said "On two occasions I have been asked, 'Pray, Mr. Babbage, if you put into the machine wrong figures, will the right answers come out?' I am not able rightly to apprehend the kind of confusion of ideas that could provoke such a question."
