Speaking: to read English books/articles aloud, listen to English radio and do "shadowing", meaning that you speak the same exact words following the radio (can be TV shows, movie, or anything) speaks. Talk with native English speakers - works the best but it depends on who you are talking with.

Writing: try to write as correctly as you can with your best knowledge of grammar, even a three-sentence journal everyday.

Making same mistakes: write it down on sticky notes and put them anywhere convenient - by the computer screen, bathroom door and mirror, etc.

Hope this helps - I am an ESL too and I know how hard it is!!

Many cities around the world have classes in conversational English that are reasonable.

Your instructor will lead classes with questions like "What do you like to eat for breakfast?" This is much better than reading a book.
