If you own a domain name (www.domain.com), but do not have hosting, you must create an account with a hosting provider (something like lunarpages, easycgi, godaddy, etc). Then you need to make the site's content, and upload to the hosting provider. If your hosting provider and the people you got the domain name from are different, you must contact the people who gave you your domain name and have them update your DNS records to the ones provided by your host.
For example, lets say i got a domain with GoDaddy, but i do my hosting with EasyCGI. I need to login to GoDaddy and edit my DNS records for domain.com to point to the IP addresses that EasyCGI provided.
DNS records generally take 3 days or so to propagate, so you won't immediately be able to type in domain.com to see your site.

There are no exact steps. There are guidelines though.
First you need to have a server that hosts the web set, then you design a web site and post it. I recommend APlus. They have discount dedicated servers from time to time that cost as little as $49/month and have excellent customer service.
You can write the HTML by hand, but that method is decreasingly popular due to more advanced and complicated web sites.
You might use the more popular web site design programs like Macromedia Dreamweaver or Microsoft Front Page. I've used both and both are very good programs but it depends on how you want to use them. Microsoft Front Page is easier to use, but makes web sites look choppy to me and are not very cross-platform compatable. Macromedia Dreamweaver can create pages that are totally compatable across any platform so it's usually considered much better overall, but it deals more directly with the code.
Personally I'd recommend Macromedia Dreamweaver (now owned by Adobe) if you want to make a good page.

You should have some good web authoring software. Additionally, will this web site be used for business and accept payments? If so, that will entail adding additional functionality.
If it is only used for information purposes, you can start to think about the design of the site. It should be easy to navigate and no item should ever be more than three clicks away.
Also consider key words when you set up your site. These are critical for pushing people your way.
