If it was accrued as party of your military duties, the result of any direct orders, or aggravated by your regular duties or orders then it can plausibly be service related. Here are a few examples:

1) Bad case of the clap - Not service related
2) Cancer plausibly caused by exposure to cleaning chemicals - Service related
3) Foot injury during PT - Service related
4) Breaking your melon because you didn't wear a helmet while riding a motorcycle - Not service related
5) Catching pneumonia during a field excercise - Service related
6) Genetic defect causing big ears - Not service related
7) Genetic defect aggravated by doing PT - Service related

It depends on the reasons behind your hysterectomy. If it was voluntary surgery or routine, it probably won't be counted as a disability. It never hurts to file for disability though and get a few % for other benefits other than cash (like easier loans and more tuition assistance), I'm not sure why filing your medical records with the VA isn't standard out-processing.
