I would tell my parents and my best friend, simply because I would want the same courtesy from them if it were the other way around.

I would tell them so they wouldn't find out after I was gone that I knew and didn't tell them. Then they would never know why I didn't do that, etc...

I don't know if I'd tell anyone. I wouldn't want my last days spent in saddness.
