First of all just his age alone I would be concerned. Just simply explain to him he could very well have an ulcer brought on by stress from running the restaurant; tell him if he would simply go to the doctors and possible have that confirmed that now a days they have good medicine that could aid in that department, but as far as the pain in the ribs and going up to his shoulder This is one thing I would definately have checked out for these pains can be affiliated with a number of heart conditions.

I think you grandpa needs to get a general check up which includes x-rays and ultrasounds of all the back and abdominal area to find out exactly in which part of the body the problem is in so it can be treated appropriately. Almost all diseases are curable if detected on time. Find a way of convincing him to get checked and everything will be ok!

What side is it on? Left side could be pancreatitis, and
an ulcer...then right side could be gall bladder.
It might be a kidney...tests for these problems
are easy! Just go in and get work up.
Even if it comes back normal doesnt mean there
is nothing going on though.

its actually really hard to tell from what is explained here , noone can really be sure unless you get an xray and an ekg , etc. , best thing to do is go to the doctors to get it looked at , but from what i get from this im thinking its a pinched nerve in your back , but since you didnt say what kind of work you do , gender , age , etc. , impossible to diagnose over the net.

Which side??? If it is the right side.... does it hurt more after a fatty food meal? I think you have gallstones.... go have it checked out by a doctor.
