The key to a great gift is thoughfulness and surprise. Effort, the time put into it, (and money) depends on you're MUTUAL connection with her.
So, first, the answer depends on how long you've been seeing her and how close you are to her.
Generally, if you're BOTH not totally in love, and/or haven't been seeing each other that long... don't go all out.
On the other hand, if you've been together a long time, and/or you BOTH (note.. the feelings HAVE to be mutual) love each other, then feel free to spend more time and effort (money isn't a biggie here so it depends on if you have it or not) into the gift.
As for thoughtfulness and surprise, BE CREATIVE.
Remember, don't waste time spending money on something that'll be gone in a few days (like food items or flowers). They can add to the whole gift but try and get something she can keep so that'd everytime she uses it, she'll remember you.

A heart locket with your picture in it, have it engraved with her initials (you can buy heart locket at Walmart, they have silver at affordable price or 10kt gold for a little bit more and they have many designs that you can choose from) one dozen yellow roses and a romantic birthday card. On the card write something like the day you first met, what you like about her, etc. Simple but something really sweet. Goodluck to you and a happy birthday to your girlfriend.

Tiffany & Co. always works. My girlfriend loves it. But sometimes you can really just make something from the heart. I know how first grade that sounds, but its totally a winner. I kept our tickets stubs from the movie that we watched together, it was the first time I said I love you to her, and put it in a frame that had love you phrases written all over it.

Without knowing your ages, and how long you have been together...go shopping with her and secretly pay attention to what she raves about. Buy it for her later. It will be a nice surpise and she will love it that you pay attention to what she likes.
