I would recommend you to be polite, but straight forward, though there are a few other things to consider:

Can the company afford to pay you more at this point?

Is your performance deserving of a raise and has it been like that for a long time?

Don't bring it up at any moment, try to bring it up when you are alone with your boss, and try to pick a time when he/she is in an extremely good mood, especially if he/she is very happy about somehting related to your performance.

Don't be cocky, just put the subject there, be humble and explain you are very interested in improving in the company.

Has it been long enough since your last raise?

Its up to you my friend how are you supporting your company the smell of fragrance that you are in your company to managing all kind of duties. You must sure to fulfill your requirements., and how much could you earn for your corporate. That much you can ask with pride.
