depending on what type battery you have they tend to develop a memory so to speak. so they retain less and less of a charge. The newer batteries we suppose to eliminate this. Previously there was a depletion charge depletion charge routine you could do to restore your battery to a longer life span.
Basically though batteries last 6 months or so then its time to buy a new one.

How long time have u used your laptop? Normally after 1 yr, the battery will got worn out.
ANother thing is that do u always over-charge your laptop? I mean do u on the power after the battery is full? This is harmful to the battery and reduce the life of your battery.
Switch off the power immediately after the battery is full, see whether can improve the situation?

I've been told that if you keep a laptop battery plugged in longer than needed to charge it ruins the battery. At first I didn't believe this either until I forgot to unplug my laptop ONE night and now it does the same as yours. My laptop is less than a year old. As to why keeping it plugged in damages the battery I am not sure why.

You prob need a new battery, if you charge your laptop too much (yes it sounds stupid) it gets almost dependant on the charge.
If you get a new battery try to keep the laptop away from the charger and maybe also try to only charge when the battery is low. Hope it helps.

You can only recharge a battery for so long and then you have to replace it. Hopefully you bought a service plan that replace your battery for free.
