Concealer's ingredients is much more dense than foundation, meaning that they provide thicker converage.
You can use concealer to cover spots, scars, dark eye rings etc but you wouldnt be able to do that with a foundation unless you use a thick layer which a foundation isn't intended for. A foundation is used for the whole face, as a base for your other make up that you would be putting on, such as blusher, eye makeup etc.
First, you dab on concaeler and blend it well before putting foundation all over. Of course, if you have beautiful skin, you can skip the concealer and just use foundation to even out the skin tone or you may just need a conealer to hide occasional zits, dark eye rings and skip the foundation if your skin tone is even.

Foundation is a bit thinner and used all over the face to even out skin tone and provide a base for the rest of your makeup. Concealer is thicker and a "spot treatment" for blemishes, dark circles, etc.

As the name implies, concealer conceals/hides things such as blemishes, dark age spots and undereye circles. Some concealers also heal blemishes (it will say so if it does...look for one w/ salicylic acid). Most people will only put concealer in a few spots (e.g. over blemishes, undereye area) and most people put concealer on first.
Foundation generally goes over concealer (if you use a concealer) and you put this all over your face. Purpose is to even skin tone & create a base canvas. Many foundations will also have SPF protection.

Concealer is:
