In my Medical Transcription Course it says to say he holds "a Ph.D."

The correct way to say it is that one holds a Ph.D. If you want to indicate from what school, you'd say that one holds a Ph.D. from XYZ University.

The KEY is that Ph.D. is the actual abbreviation for Doctor of Philosophy, just like B.A. with the periods is the proper way to say Bachelor's of Arts.

"a Ph.D." would be correct. Ph.D. is doctor of philosophy. Would you say someone holds the doctor of philosophy? Doesn't sound too good, does it?
