If you were at a pool or lake or river and saw someone swimming with jeans, shoes, socks, and a long sleeve shirt on, what would you think about them? Just that they're weird? Trying to avoid UV rays? Or would you not think anything of it? Or something else?

I would admire them for their joie de vivre. But, come on, take the shoes off! :)

I would wonder if they are trying to do themselves in.
If I could engage them in a talk, I would try to find out what was going on.
Or maybe they are trying to rescue someone. The possibilities are endless why they would swim fully clothed.
Those are probably the 2 things that would pop instantly into my head.

I would think:
"Hey, they're swimming fully clothed! Then something along the lines of:"Why is there that stinky looking green mist around his feet?"

he fell into the pool and he's feeling embarrassed. he felt bored, so he jumped into the pool completely clothed cos he didnt have his swimming trunks and he cant bypass swimming bare the two.
