a leak if in the vehicle that long, but you should'nt be keeping it in the vehicle for that amount of time. you should be changing it out once per year.
it's a fact that some will go down due to evaperation by it cooling off a very hot engine.

No body uses a 100 percent mix, the water will eventuially evaporate, but could be a small, minute leak!!! also not a good idea to go more than 2,3,years between changes, looses its, properties, also a lot of conditioners stop working

Anti-freeze/corrosion inhibitor based on ethylene glycol will get used up in preventing corrosion and will loose it's potency.
In general the cooling system should be flushed every 2 years and re-filled with new coolant.
It is absolutely crucial that different anti-freezes are not mixed, use the same brand. UV will destroy any pre-mix that you might have for topping up, so store in the dark.

Yes, it would evaporate also it would be used up through the running of the vehicle. Add some more, your vehicle should have a check over every six months. Winter and Summer for the saftey of the vehicle, your self and others in the vehicle and on the road.

In your radiator for 5 years?
Too long.
As for leaks, this would be visable under the radiator on the ground. It will be color of the fluid you use.
Get rid of the old fluid. Flush the radiator. Check the core for leaks, after that long.

Leak if there is a lot missing. If it is just a little, you may have lost some from expansion when it is hot.

When the antifreeze is hot it will flow into the reservoir. from there it can evaporate.
