How much does a slab foundation clost?

Question

for a 2000 sf house, assume $100/yard for concrete, or how do I roughly estimate it? What else do I need to ask or know?

Pad thickness and what is to be used for reinforcement. (re-bar is expected to already be in place and ready to pour when the truck arrives.)
Also need to know the mix. Just ask what's available and have the salesman explain the differences.
