I've never heard of a form to outline, but there is a form to perform that action. I know because my father adopted my nephew temprarily as his guardian while he was living with him.

There is an online form for temporary guardianship (I'm not the one who found it, so you'll have to do a bit of keyword searching). Someone I know just printed and signed a 6-month one recently - she filled in the 6 months - the form allows you to enter whatever amount of time you'd like to.

Yes there is - it is called Power of Attorney. It basically will say that the guardian will have the right and be able to make decisions in things important like medical care. You might be able to get one, from a book of legal forms from a place like OfficeMax, or there is software that has legal forms on it. Barring this, I would recommend you contact a lawyer. If someone is letting there child stay with a relative in case something happenes someone should get a noterized piece of paper with all the details on it and in case they should have something stating they make the decisions for the child in the present moment...

Your attorney would know. . .just make sure you cover medical liability (in case the child gets sick or there is an emergency) and make sure you have IN WRITING who is agreeing to what (finances particularly) including discipline, traveling, schooling, church, anything that affects you and the child. You want no loopholes that you or your family could be stuck for. Unfortunately, this happens on occasion.
