Are you speaking about a genitalia item? If so, did you sign any papers stating you could not have another operation or sue because of future ailments or parts returning? If not, then contact the surgeon or hospital that performed the surgery and go from there. You should see a doctor, and your questions would be easier to follow if you added more details instead of posting another question, and gave all the details (ie what was wrong when you were 15, and what the problem was). Honestly, the only way to know is to contact your doctor, asking on here will only give you a guess as to whether you're pregnant, or if there is another problem....

your not really telling use nothing expect that your not 15 your 20 and that you had surgery and you asked us how can it happen twice
