When you apply for your wedding licence at your local courthouse, they'll give you a form to fill out and send to the social security office. After you're married, you send in that form with an official copy of your birth certificate and an official copy of your marriage license. (I think they may have also wanted another form of id - like a copy of your driver's license.)
They take care of it and send you a new social security card, and they return your other official documents. It takes about 6 weeks after you send it in.
(Oh, and it doesn't automatically change everything. Once you get the new social security card, you'll have to take it to the DMV to change your license, and to all of your doctors to change your medical records, and to all of your banks to change your accounts. Bring the marriage license, too. Some of those places have a lot of hoops to jump through.)

You need to get a marriage license so then you can choose to change your last name, have both names or save your last name..

when you sign the marriage license, you sign first with your maiden name then you sign again with your new name, take that to the DMV and social security office and your name is changed on your license and social security card. unless you wanna keep your maiden name...
