The nurses usually take your vital signs, any changes you've experienced lately, whether or not you gained/lost weight, symptoms you're having, meds you're on. They fill out the H&P for the doctor. Nurses get everything ready for the doctor.
The doc comes in to check things out and afterward, they will dictate a letter to put in the chart and to send to any other doctor you have (specialist, primary, etc). On the H&P, the docs will note what their diagnosis is and what meds you are being prescribed or what surgery you are to have, etc. That way, they can browse those two things before going in to see you in the room.
That's about all the docs do. Unless they learn something they feel is of importance during your visit. Then they will document that under the Notes section.
Any little thing must be documented in the chart though. If the receptionist tries to call you to schedule a heart cath and left a message, they need to document that. If you have an abrasive personality, they must document that. If you mention that you faint when you have your blood drawn, that must be documented as well. All this is supposed to help better serve you, the patient.
