Are you asking what happens to the muscles, tendons and ligaments, or how to treat a twisted ankle?
Physiologically, a twisted ankle usually means stretching, pulling and sometimes tearing the soft tissues in the ankle.

Treatment involves rest, compression, ice and elevation of the injury. Get off of it, wrap it with an Ace bandage , ice it for 20 minutes of every hour for the first 24 hours, and get it elevated above the level of the heart.

It is very painful indeed, but well it happens so if you want to be back walking again try to give it some rest; and take some inflammation medicine and you can also put your ankle in a bucket with hot water for five minutes and then change it to one with cold water... repeat it for an hour and... You'll be as good as new in 2 or 3 days.

Ice it 2 or 3 times a day to keep the swelling down to allow it to heal quicker

For the first 24 hours, keep it elevated and put an ice pack on it to keep down the swelling. After the first day, still keep it elevated, but use heat to increase circulation. Cold will retard healing after 24 hours.

rest it, put it up, and put some ice on it, don't wear any shoes with heels for a while since it will be weak for a bit and prone to twisting it again... feel better!
