Certain high frequencies are difficult for your brain to handle and interpret. This is because it is getting close the spectrum of sound that it cannot hear. If it were a more steady sound like a high pitched beep it would not be as bad, but the uneven warbling sound it makes it harder for the brain to interpret.

The high pitched frquencies that are produced during the interaction of the fingernails and a chalkboard cause painful surges due to the fact that most of the human race cannot stand high pitched frequencies.....Supersonic for an example

What about the feel of chalk...that is worse than the noise.
I always wore gloves when I had to go to the chalkboard
