A good apology letter should:

1) Start with recognizing your wrongs. I did A B and C

2) Should sum up why I now understand why A B and C is wrong

3) Take in account the feelings of the reciever of the letter and (this would be a good point for the actually apology) "I'm sorry for A B and C, as you can see I understand now that doing A B and C was wrong and I really understand that it hurt your feelings"

4) End the letter by offering what can be done to fix the damamge. "I know you are hurt/upset/mad and I am sincerley sorry, if there is anything I can do to help fix this please let me know.

5) The final part of the letter should end with your feelings. "I Love You & I'm Sorry" "I Truly am Sorry" "You really do mean so much to me, I'm so sorry"

I'd recommend clarifying the purpose of the apology. Standard letters of apology are typically used for a variety of business purposes, and one you might use to apologize for missing a job interview is not the same one you might use to apologize for crashing a bus full of orphans.
