i wasn not able to give my exam on a according day for my mistake. on make up exam i also wasnt able to?

attend on the class bcus i have to attend other classes and i forgote to inform about that to the teacher. But my teacher allowed me another time to give that exam by submitting an application(bcus wasnt able to give the exams before).

Now i sit on my chair and cant get that what shud be written on the application..?should it be like this- " i wasnt able to attend the exam for *********************
if i write this. then what should i write on the next line as a reason?

Really?? You probably need to reexamine your priorities. One can understand how you could miss the exam once, but the makeup exam too?? Either you are game playing or you demonstrate irresponsibility. In either case, you need to get your act together.

I hope that exam is not in English spelling and grammer. It would be better if you don't take that exam. Tell your teacher you need more time to study.
