She does her job really well. What do you pay her for? Coming back and training with her should be your thank-you.
Congrats to you for wanting to lose. You deserve most of the credit. She couldn't make you lose.
I'd just keep it short and sweet, and say, you do your job really well, and I appreciate your helping me see that I am in control of my weight.

Dear Trainer,
Thank you for helping me lose 50 pounds over the past year.
You don't have to use those exact words, but it gets the point across.

Just write what comes to mind. Tell her how she changed your life, and what it would have been like if she had been absent from it. Let her know she made a difference and that you are sincerely grateful for that. A good line no matter what your situation is "I'll never forget how you've helped me."

Wow, you did good girl! And of course the trainer helped, but in the end it was you who did the hard work.
Thank him/ her for the support and ask for a maintenance program, since in all honesty; you want to keep this weight, don't you?!

Tell her all the new positives about your life now. Encourage her to keep on doing the same for others. Let her know how important her job is.

You needn't be elaborative in writing a thank you letter, a simple one with words from the bottom of your heart would do the job .
