What is it and why would I need one?

A stabilizer plate that makes the boat PLANE..Shaped like a sharks tail or just a rectangle plate that levels the boat. When you throttle up a boat , it Tries to "nose up" at an angle the "Plane" or "Trim Tabs" level and stabilize the boat.

Everyone rigs their boats a little different. By adding fuel tanks or Livewells Batteries etc... this increases the weight and handling characteristics. These changes could make your boat "porpoise"
which means simply bobing up and down OR your bow sticks up so HIGH you think you will never get it on a plane.

Outboards and outdrives are not built with any Plane Plate - as you say..
I have never heard anyone use this exact term.
You may be referring to an After Market add on item for stabilization - designed for keeping the prop in the water at different angle - which thereby changes the pitch of the boat and raises or lowers the Bow...
There are many built and have many names.
Sting Ray and Doel-Fin hydrofoil stabilizer are most common.

Your other choices for stabillity and performance are:
Hydrolic Transom Jack Plate
Power trim and Tilt
Trim Tabs

if your refering to what ive always known as dolfin. then dont waste your money.
they are good on small outboards with small boats, and im talking small like 10foot boats with a 5hp motor or what not. otherwise, in my experience, they have all been a waste of time and money and nothing more than added wetted surface area producing drag.
