Well, it depends on the level of reality from which you are looking or speaking. From the level of everyday mythology and propaganda, the "people" make the ultimate decisions. From the level of some religions, god (or the devil) makes the ultimate decisions. From the Constitutional point of view, it's the Supreme Court that finally decides, though Congress has the first say so. From the administration point of view, it's the president. From the economic view, its the central bankers who make the ultimate decisions. For the conspiracy theorists, its the Catholic Church, or the Learned Elders of Zion, or some hidden, secret society of one world goverment. Still others might say it is aliens from the planet Zonked Out.
Deciding who makes the ultimate decisions is as slippery as jello, and just as hard to hold onto. A lifetime of research probably wouldn't answer the question. It may be some evil robot hidden under Mount Rushmore, for all we know.

That would be Congress.

In general the same people that have made decisions since the fall of Rome. A small elite group of the worlds aristocrats. Europe is much more aware of them and manages to maintain better conditions for the working people.

Look at the president and his entourage. All of them involved (or previously involved) with big oil corporations. Oil corporations make the big decisions. Directly or indirectly.
