Just give your full name, the address, I would write like To whom it may concern: then give that info, tell them you intend to vacate the premises on what date and your new address to send the deposit afterwards. That should be enough info for them. Just keep it short and sweet.

If you are not on a lease, then you respectfully state that you are moving out in 30 days. Mention any comments you have had about your stay, and a general reason why you are moving out. Make sure you are nice, as you will have a better chance of getting your deposit back. This letter really only needs to be a few sentences long, so don't worry about making a big deal out of it. Just make sure your nice. I've done this four times, and it's worked for me. This letter also gives them a heads up on what they should have you do next before departing. If you are on a lease, however, and your lease is up, make sure you come up with a good reason: e.g. My husband is going to war... something serious like that, or they won't let you out without a hefty price.

Not all apartments have this notice. Are you breaking your lease? If you are then you need to see if the reasonf for you moving out are valid, otherwise you can be charged for the remaining months. If you're not breaking your lease then you need to state the intent to vacate by ie: August 10, 2006. Leave your forwarding address on the letter so they know where to send the deposit. State the date of the letter, your name, your last day of residence, your forwarding address and your phone number. That's pretty much it, unless you had a great landlord then you tell them so and tell them why your leaving. If you didn't have a great landlord, you don't need to be personal in your letter and you don't need to leave your phone number with them either.

You just have to let them know when you'll be vacating the premises, that you expect your security dep back, and give them an address to send you the check, and any future mail or correspondence. If there are any open issues with the landlord (such as bill for repairs owed, etc.) you need to resolve that in the letter, and before leaving.

If they dont have a form it doesn't matter what you say as long as they know they have 30 days and you are out of there. Be sure to pay rent or any fees from breaking a lease!
