should it be (I, S.M.Uzair, am ........) or (I, S.M.Uzair, is ........)?

It should be "I, S.M. Uzair, am" because I is the subject of the sentence while S.M. Uzair is an appositive. This means that the name modifies "I" and "I" being the subject must make sense with the verb. Therefore, if you delete the name the sentence must still make sense.
