depends on the type of filter..... I would start by reading the instructions and it that doesn' t tell you where to fit it then give the manufacturer a call... failing which put some more information here

First of all, what type of filter are you talking about, there are a hell of a lot of different filters, some go under the water line, some go above the waterline, and some go under, check and see if your filter runs dry above the water line then put it on the water line, if it still runs dry then it has to go below the water line, just play around with the filter you will soon see where it belongs.

A filter is for cleaning the water so it should go below the water level, above the water level will be no use.

Does not really matter. However, the filter should break the water surface to add to oxygen exchange, where harmful Co2 is taken out and oxygen added. But, internal filters under the water line, cannister outside of tank and under water line, powerfilter, roughly level, and any other internal filter inside

Assuming it's an internal, where the output is depends on the depth of the tank vs filter size. My fluval 4+, for instance, has the output above the water becasue of it's size, whereas the 2+ is completely under water. Wherever it is, it should be placed to allow good surface agitation.

filter should be below the water level .
ideally suspended in water inch or two above the bottom of the tank or resting at the bottom

normally they go below the water line

but check they manufacturers instructions
as to how far and where some go in the water some go outside the tank
