I think the one that sticks out the most was in Mighty Ducks 2 the kids steal the Zamboni and drive it through the boards. When the hit the brakes on the ice the tires squeal. This would not happen on ice.

Star wars - when entering the room where c3po and r2d2 are (durring the garmbage masher scene) one of the storm troopers bashes his head on the door as it goes up.

"Top Gun" Tom Cruise flies his plane upside down above another plane. It's not the Blue Angels. His plane should have began to descend, as the wings are not designed to maintain altitude upside down. The scene is based on an actual dogfight, but those two planes were in a vertical climb, not flying level.

"The Wizard of Oz". InThe dance scene with Dorothy & the Scarecrow, they had to retake parts of the shot a few weeks after shooting the original. Watch closely and you'll see Dorothy's hair go from short, to long, to short again a couple of times.

The most famous mistake in my mind was in Plan Nine from Outer Space. Replacing Bela Lugosi (who died) with a guy that was a foot taller.

its not a continuity error, its a mistake by a character in a movie -

that dude that failed to shoot down the escaping droids at the bagining of star wars episode 4 "a new hope". that dude musta been kicking the s*** out of him self when he saw the reprecussions of that blunder.
