It would depend on a lot of factors... A mother who goes into labor knowing she is having twins and has them very close together would probably find the first one more painful. By the time the second one is born (again, just a few minutes later), she's already had all the stretching and is maybe a bit numb from it.
A mother who didn't know she was having twins would probably find the second one more painful, just because it was beyond her expectations.
And a mother who has a break between the two births might find the second one more painful because it follows a break rather than following right after a lot of more painful contractions.
However, I should note that *most* women don't find birth to be painful at all. Labor is painful for almost all women, but the birth is often *not*. So the question is likely to be a moot one anyway.

Man, you've only got a couple of real answers with this one.
After having given birth to 5 children, and am now waiting on my unborn twins' birth...
I'm sooooo very glad that I'm alive in a day & age where we can ask for medicine to help us get through the pain.
They both will hurt. And when the time comes, no one's going to gauge the level of pain, or the difference in pain between the two. I've been informed of the possibility of a breached 2nd baby, been informed that I might have to give birth feet first, or have a C-section. But, I can only hope...that the anesthesiologist that I've always personally requested for - will again be my saviour and make it all a little better...

Well for me it was the second one! When it was time for the first baby to come it went fast and easy, but my second daughter ended up breach and was a lot more painful to deliver. My twins are now 5 and I think their birth gave a lot of insight on how they would be as they grew because my first is easy going and can easily get a long and the second one kinda has a mind of her own and likes to get her sister in trouble. LOL...its been over seven years since my last set of twins..ive had two sets...It took more to get the first twin out as far as pushing went...but once I gave birth to the first one just the thought of having to do it again was a bit overwhelming...but the second one came out on just a couple of pushes...I did this all with absolutely no pain killers what so ever. Which does sound extreme but the good side is that once its all over you feel terrific. I think overall they were both just as painful....

Most likely the first since the second is more likely to just fall out after the hard work is done. That's if it's a vaginal delivery, which in multiple births, isn't likely.

I don't know about twins, but after giving birth to my 3rd. child I thought I was giving birth to another baby because of the pain of the after-birth.

Well,i m way too young to have children but i would guess the second. Thatz because u have already pushed one out and know how it feels. Therefor it would hurt more because u r scared. Plus u still have to push out the plecenta and with my mom having just one baby she had to get stitches.Most of this information came from watching a real birth my mom had.Hope i helped!

it all depends - for me it was the second - and my delivery was all natural no drugs - I didn't get to the hospital till I was 8 centimeters dilated - never knew I was in labor - my daughter was out in about 4 pushes and my son was breached so it was very painful - both were delivered without c-section and both are happy and healthy 15 month olds now - it all just depends - I noticed those of us who've had twins have said the 2nd - but I really think it all depends on the situation and circumstances of your delivery

Although it was by C-section it was the second one. Little booger must've known it wasnt time to come out yet (they were 6 wks. early) because she wedged herself up under my ribs and there was tugging and pushing and it was rough. They even had to make another incision inside just to get to her. But I'm happy to say they are happy healthy 10 month olds. :) Come a long way from the 3 and 4 lb. babies I brought home from the hospital
