My first suggestion is chiropractic care- it seems expensive but in the long run VALUABLE to your posture and overall health - the condition of your spine does not just affect posture or cause pain- it can interfere with any and every function in your body without you ever having a backache..
As far as exercises go, stand facing a corner and stretch your chest muscles by putting your hands straight out on the walls and lean into the corner, hold the stretch as long as you can and do this with your arms at various angles- you will feel it right away. any core exercises will help- pilates, core training- something to focus on strengthening the abs.
Roll up a towel and put it behind your neck, standing strainght with shoulders back, pull firmly and lean your head back and to either side a few times.
Pay attention to how you sit, stand and sleep, dont let your shoulders slouch forward - ask others to remind you gently when you are slouching as it becomes such a habit you dont always realize you are doing it. Don't cross your legs at the knees when sitting - cross your ankles or sit straight with oyur feet side by side.

Practice. Practice, practice, practice.
Walk with books on your head, shoulders back, tummy and butt tucked in.
Stick a post-it on your computer that says SIT UP STRAIGHT. Set an Outlook reminder for every 15 minutes that says SIT UP STRAIGHT. When you get to the point that you're already sitting up straight when the reminder pops up, change it to every 30 minutes and so on.
Pilates is helpful for posture, too, as is yoga. You can get books and sometimes even videos at the library.

First of all, make sure that you eat healthy, and take multi-vitamins if you can. Secondly, always practice good posture, even when you are at home on the sofa, after a while it will become a habit. Third, get regular excercise, it is good for your back. Fourth, stretch your back sometimes, that might help also. If you have significant back problems, you might want to see a chiropractor. Also, be confident.

I'd suggest taking up yoga, which will help elongate your muscles and teach you how to focus on the position your body is in. You might also want to work on your abdominal muscles, using stomach crunches, Pilates, etc., as these are the muscles that support your back. The stronger your abs, the better supported your back, the better your posture is overall. You can get Yoga and Pilates videos for free at most public libraries.

stand up against the wall with your heels against the baseboards and your shoulders touching the wall. Practice this daily 10 min. at a time 2-3 times a day. How old are you, if your middle age or older the signs your describing could be Khyphosis and nothing past surgery if even possible could help or you could have some other spinal condition. You may want to check with your doctor.

i think the best way to improve is by sitting,standing straight and by doing some simple stretching exercises.By correcting yourself, you can do this by telling your friends and family to correct you.You dont have to strain yourself by staying erect all the time, just stay relaxed and focus on keeping your neck and backbone straight.You can also try and change your chair (wherever you sit for long hours).So just observe your posture,try to keep your body straight and it'll be second nature in no time.

Take in a deep breath, from your stomache,& practice pulling your shoulders up,& back, at the same time, this will change your posture if you do this daily.

Always walk with your shoulders held back. Take broad steps when walking. Also try balancing a book on your head. Buy some insoles for your shoes, when your feet are taken care of you have more energy to stand up straight. And always keep your head up!!

my old volleyball coach once told our team:
theres no good excercises for posture, you just have to do it. be the tallest you can be, pretend there's an imaginary string that pulls you straight up, pretend there's a tiara on your head, whatever works!!!
and what else motivates: you look skinnier and more attractive when you have good posture!

Sit up straight all the time; make sure that your shoulder blades are touching the back of your chair and that you are not slumping, especially when sitting in front of the computer for long periods.
