Lieutenant is actually correct surprisingly - see below.
It's All in the Game, written by star Peter Falk, with Faye Dunaway as
the designated high-society adversary, may not be the most brilliant
episode in Columbo's 62-show history dating back to 1971.
But it probably belongs in the top 25, and besides, even on a bad night
(which this isn't) Columbo provides the pleasure of witnessing one of the
classic characters in TV history.

Falk clearly was in a puckish mood when he wrote It's All in the Game. The
Columbo formula normally pits the dowdy detective against a well-toned and
tanned specimen of Tinsel Town's power-lunching elite.
Screenwriter Falk pokes fun at his character - "You know, you're not
exactly Robert Redford," Black quips - and some Columbo conventions.
"What's your first name? " Black asks.
"Lieutenant," he says, declining to end a 22-year mystery.

The character never had a first name on the series. Lieutenant was his rank and not his first name.
Some websites says "Phillip" but if you check out the second source and look at the ID from the episode of "Dead Weight". I swear it looks like "Frank Columbo"
Until then the correct answer is no name is given.

It was never mentioned in any of the series but in the first series he showed his police badge and the camera shows it as saying Frank Columbo

No one ever knew Columbo's first name. It was never revealed in any episode.
Philip was suggested by Trival Pursuit but taken back when challenged.

As you can see, there is no real answer for your question. He doesn't want you to know. Peter Faulk(his real name) was great in the role though. He could drive anybody nuts.
