There's usually a co-director or asst director. The official director will help set up the scene & review it when it's finished and involved in the final editting. For more info, I'd recommend watching a director's cut of a DVD...maybe on of Clint Eastwood's movies.

How does he "GET TO" or how does he actually manage to do it?
I'm sure he doesn't yell directions or ideas to himself out loud. He probably just acts on them, and takes others' input.

in big movies there are executive producers directing agents and camera operators and filmographers cinematographers and many other people capable of taking the production lead!
