Residing in the state of CT where do I send a request for the current guide LMRP/LCD??

currently receiving medicare in the state of CT. the guide that I am requesting is called an LMRP/LCD guide. Requesting what address do I send my interest in receiving that guide? Thank you.

Go to the Centers for Medicare & Medicaid Services website here: http://www.cms.hhs.gov/CMSForms/ When you get there, type in LMRP/LCD in the "Search" box. Tons of forms related to LMRP/LCD will show up. Click on whichever one you want, and you can print a copy online. If I had known what you were looking for exactly I could have found it for you...but it's not going to be hard for you to search.

Medicare is a federal program, so I don't believe that you need state-specific forms.
