Your first name should go first - that's your given name, the name that is NOT shared by your brother or sisters! Maybe that's thileeban, I'm not sure. It's the name that your mother would call when she wants you to come to the dinner table, or maybe your friends would call you this name.

Your last name - also known as a surname - comes last. This is your FAMILY name, your father, your sisters and brothers, and your grandfather should all have this same family name, and in most countries your mother and grandmother would have it too. This name usually changes when a woman gets married. It changes to her husband's family name.

If you have a middle name, that would go in the middle.... but if you don't, then you will only use YOUR name first, and then your FAMILY name last.

It's difficult to explain this to someone who isn't fluent in English, but I'll give a few examples of famous people;

Velupillai Prabhakaran - Prabhakaran is his family name, so it comes last. (Although Interpol say that Velupillai is his family name, so it would be the other way around - I don't know which is right though, but Interpol are the only ones who say that!)

George Bush - Bush is the family name.

Ronald Reagan - Reagan is the family name.

Brad Pitt - Pitt is his family name.

Indira Gandhi - Ghandi is the family name.

Frst name, then surname, which is the same as last name. Father's name may be used as middle name. You are Thiliban Ratnam Durai.

In the English language and most of western culture, it is first (personal) name then last (family) name. Surname is in fact another word for last name. It is the name passed down through the generations on the father's side.

Sometimes first names are also passed down, but in that case it is Charles senior, Charles Junior, Charles 3rd, etc. All of them would have the same last name, and the designation senior, junior, etc. would be so people could tell them apart.

In Chinese and related cultures the order is reversed. Zhang Ziyi (actress in "Memoirs of a Geisha" among other things) for instance, where Zhang is her family name/surname and Ziyi is her "first" or personal name. She reverses it to avoid confusing western audiences.

Last name and surname are the same thing.

Surname is NOT an initial, dim bulb. An initial is the first letter of a name. That's what makes it an initial.

"First Name" is sometimes also referred to as "Given Name". "Last Name" and "Surname" are the same thing and mean your father's last name, which although she may not necessarily use it, is also referred to as your mother's married name. As to the order, it depends. For filling out a form, read carefully and enter the correct name on/in the correct line/box. In terms of how you refer to yourself, that is up to you and your cultural/ethnic/social conventions.
