When trying to conceive you are better off having him stay inside you for a bit. Just so it has a chance to travel upwards instead of coming out with him.
As for the pillow 1 or 2 is up to you. All you need is something to tilt your pelvis upward.

Ah, the pillow-under-pelvis, also know in the south as an "assjack". Well, that would depend on whether you intended to put your face on it and sleep afterwards, now wouldn't it. I'd do the same I do any other time in terms of keep-in/pull-out, but if you want to put your face on it, I'd use a towel!

you can keep it in or pull out, but there is not a difference, and if you are small you need 1 pillow and if you are large, then 2 will do the trick

Does not make any difference. No pillow needed, doctor does say to lie still for a few minutes.
