I don't know if they have accidentally become untethered, but a number have intentionally gone without a tether. They were using a kind of jet pack to maneuver.

Back in the early days of the shuttle program, there was a piece of equipment called the Manned Manuevering Unit which was basically a rocket pack that allowed the astronaut to move around untethered. It was used for three shuttle flights in 1984, but was discontinued after the Challenger disaster in 1986, after it was deemed too risky.

"become untethered"? Depends...
There have been untethered EVAs.
The first in space was in 1984 (STS 41B).
The first publicized untethered EVA was Neil Armstrong in 1969.
