Here is a possible explanation:

The foreground objects (earth, astronauts, shuttle/space station, etc) may have been lit directly by the sun. This would make them *much* brighter than any of the stars. Since the camera would select an exposure level suitable for these foreground objects, the stars would become basically invisible.

This is similar to why you don't see the stars in the daytime. Same atmosphere, same stars in the sky, but you don't see them because everything else is so bright. The infinitesimally extra amount of light from the star is tiny compared with the average brightness of any particular patch of sky.

No. I have watched quite a bit of it and never saw the stars, even on the night side of Earth. The camera is just not sensitive enough to see the dim stars. You need special night vision cameras that are extra sensitive to dim light to capture the stars. They used such cameras during the gulf war, in 1991, so that you could see the stars over Baghdad while the antiaircraft guns were firing. But such a camera gets blinded by bright light, so it is never used if any bright light is expected.

The bright suits of the astronauts reflect a lot of light, which makes it tough to see the stars. In addition to that, the exposure time may not be high enough.

Yes you can! It depends on what direction the camera is pointed in. I've seen some beautiful shots of the stars... those astronauts are so lucky... I'm jealous.

Because some idiot in Hollywood forgot to paint them on the backdrop again! IDIOTS! We need to write to their union about this....

It's all filmed on a back lot anyway. The space shots are actually filmed underwater - the astronauts are "floating" in water.
