Terminator 2 - (Robert Patrick) - when the T1000 is chasing after the good guys in the helicopter he's shooting a gun with both hands and another hand can clearly be seen controling the helicoptor.
I'm sure there are lots more, but those are two that I can clearly remember seeing in the theater
(oh and the ghost from Three Men and a Baby is actually a card board cutout that got left in a scene - I think it's Ted Danson)

Yes, one very scary one in 3 Men and a baby... something like that, I dont really remember the title that well. But, when they were walking around there was a dead girl in the blinds that wasnt susposed to be there... Freaky!! Sorry I also dont remember the actors or actresses.

My dad actually noticed in the movie "Days of Thunder" when Tom Cruise and Nicole Kidman go see the guy he had the crash with, the guys girlfriend turns and says "Hi Tom" instead of "Hi Cole". Later people started noticing and was even played on VH1

Titanic- (when they have Leo Dicaprio in the lower part of the boat and he is cuffed to a pole, there is water higher than the windows (port holes) and the port holes are open, but no water is coming in...
Pirates of the Caribbean- When Johnny Depp got hand cuffed, then he took Keira Nightly hostage for a couple of minutes, when he is swinging around in the air to escape, he uses his cuffs (that are unlocked) to go around a rope and swing down and then when get gets to the ground, he is magically hand-cuffed again.. and has to go to orlando blooms blacksmith place to get them off, when they were off 2 mins earlier in the film.

Men in black. Will Smith and Tommy Lee jones were in a car chase in the tunnel. When the car flipped over and Will smith's legs were showing they were white. I guess his stunt man was white.
