I'd offer my car to my friend so he can drive the old woman to the hospital. Then I would wait at the bus stop with the perfect partner I've been dreaming about.

I would give the car to the old friend who saved my life so they might save another by taking the old lady to the hospital. That still just makes one passenger in the car. Then I would stay at the bus stop with the perfect partner I had been dreaming about.

1: Give my phone number to the perfect partner.
2: I would then ask the friend who once saved my life to help me
get the old lady into my car to take her to the hospital.
I would hope that my potential perfect partner would respect my
decision to help someone less fortunate - moreover, giving this
person a ride would be at best a transitory process, no guarentee
that it would lead to anything romantic.
I might then ask my friend who once saved my life to talk me up
big time as I drove to the hospital...

As long as one confines one's thoughts to the strict wording of the question...which ONE would you OFFER a ride to, the situation is irresolvable in any manner with which it's easy to live with yourself. The trick is to "think outside the box".
Give your keys to your friend, tell him/her to take the old lady to the hospital and wait at the bus-stop with the partner of your dreams.

I would pull up, thank my friend hank for waiting there with my wife, give him my keys so he could drive Mrs Smith to the Hospital, all the while holding my umbrella over my wife Jodie so that she didn't get wet, and after Hank drives off with poor old Mrs Smith, Jodie and I would go back inside the resturant behind the bus stop and finish eating dinner with Laura (Hank's wife) as we waited for Hank to return with my car so that Jodie and I could go home.
