Your blade may have been thicker. Also some lawnmower nust, are reverse threaded, which means when you were trying to loosen it, you were tightening it, which made it strip the threads. Also check to make sure you have the blade in right side up. If the blade is upside down, that might also cause this problem.

Make sure that the washer is next to the bolt head, and not under the blade.
I believe the order of installation should go blade, flat washer, lock-washer, bolt. It may be coming loose if the lock washer is missing.
Maybe a new bolt may be in order as well, just be safe, nobody wants you to post the question "the blade came off again and chopped my feet off what to I do"
Good Luck

do no longer USE A TORCH. you are able to surely destroy the threads interior the bearing that the bolt screws into. Penetrating oil works good, so does a 18 inch long a million/2 inch tension breaker bar with a great sized socket. Block off the blades with slightly wood so as which you're actually not combating to hold the blades and turn the bolt. The bolt might desire to unloosen to the left, yet verify the blade course to be risk-free.

most mower blades are notched so you may just have not set in all the way, also you should use some locktight, its a liquid rubbery substance that you put on the screw before screwing on the bold and it will keep it from coming loose. You can get it at any auto or hardware store.

Blade has TWO sides to it.
If you had put the wrong side facing down towards the lawn and did not tighten the bolt properly it will come loose.

sounds like the bolt hole may be stripped. if that blade comes off during full throttle operation it could be deadly. you may want to take the mower to a specialist and have them resize the bolt hole or something. you need to double check that you put the blade back correctly and not upside down. it is possible.

Hi,, sounds like you have not put the bolt in right......
be sure to put it on with the guard on the top of the blade... be careful... or get someone to help you ....
That blade can sling out and cut you very badly.....

If the hardwaree does not match up just as it did before removal, it is wrong or not correctly installed. Check it against the old hardwar for differences. BE CAREFUL!

You may have reinstalled the blade upside-down.
