Because the said action personalizes the meeting. It shows sincerity and being one with the person they are meeting. ;)

Can't be sure but I have heard two origins for this.

1) it shows you do not have a weapon in your hand (gos back to the tims B.C.) and
2) a show of warmth -- to show reverence...when meeting a high ranking church official one would kiss his hand and kneel...shaking hands is a lesser form of recognition among laypeople -- obviously stronger when a man meets a man and much gentler when a man meets a woman.

In the Middle Ages, they offered each other their hand in order to show they were not holding a weapon. It was a safety precaution.

Dates from way, way back. A person would extend the right hand to show that he (typically) did not have a weapon.
