From personal experience, I can tell you that lesbians are everywhere, in small towns and big cities. Most of us just aren't that easy to spot, because we don't look like your stereotypical lesbian.
As for greater concentrations of them, as a rule, the larger the city, the larger the percentage of GLBT population. In my experience, gay- and lesbian-friendly cities include Chicago, New York, Miami, San Francisco, and LA... and a couple surprising ones, Tulsa, Houston, and Dallas.

I'm glad you asked that question.
They are often found in clusters of 3 or more, beneath old willow trees in elderly people's gardens. They're often chanting "Moony moony moo, cha cha galinga" (repeat) or something similar. Butterflies are frequently found nearby making love with one another, as the scent of the lesbians act a an aphrodisiac for them. I could go on, but that's enough for now.

Lesbians and gays are found all over the world. They have existed since the dawn of modern man (and woman) and will continue to exist until modern man is no more.

I think Jorge got it mostly right. They are pretty much restricted only to the island of lesbos. You cant find them any place else and the ones that leave the island immediately lose their liscense to practice. Its like gay marriage. Once you cross the strait (get the pun i hope) line you loose all your ability to be a lesbian.
